摘要: Introduction: One aspect of clinical integration involves case managers' tools and particularly the individualized service plan. Methods: We examined plan content use in PRISMA experiment. analyzed 50 charts, conducted recorded interviews regarding with all managers concerned (n=13). Results: Delays between starting management writing were long varied (0–596 days, mean: 117 days). During interviews, was described as ‘last step’ once active planning phase over. The reasons for formulating plans mainly administrative. From a viewpoint, used memoranda not to describe services (842 interventions mentioned plans) or needs (694 problems mentioned).
