摘要: Cancer cachexia, weight loss with altered body composition, is a multifactorial syndrome propagated by symptoms that impair caloric intake, tumor byproducts, chronic inflammation, metabolism, and hormonal abnormalities. Cachexia associated reduced performance status, decreased tolerance to chemotherapy, increased mortality in cancer patients. Insulin resistance as consequence of endocrine dysfunction has been patients characterized hepatic glucose production gluconeogenesis, unlike type 2 diabetes, normal fasting high, or low levels insulin. cachexia results composition the lean muscle mass without adipose tissue. Alteration visceral adiposity, accumulation intramuscular tissue, secretion adipocytokines from cells may play role promoting metabolic derangements including proinflammatory environment insulin resistance. Increased ghrelin, testosterone deficiency, vitamin D also contribute metabolism glucose. cannot be easily reversed standard nutritional interventions identifying treating at earliest stage development advocated. Experts advocate for multimodal therapy address impact reduce treat derangements, which propagate weight. Treatment critical component more research needed.
