摘要: Non-unions are a relevant medical and socio-economic problem. Hyper-, oligo- atrophic non-unions as well septic aseptic differentiated. Correct classification is essential for the selected therapy. The "diamond concept" describes five pillars, on which bone healing based that have to be considered in treatment of non-unions: osteogenic cells (mesenchymal stem cells), osteoinduction (growth factors), osteoconduction (scaffolds), mechanical stability, vascularization. Factors predispose non-union also influence fracture healing. gold standard therapy still resection non-union, decortication autologous grafting. No advantage could proven any numerous procedures monotherapy. But combination various - polytherapy seems promising. aim optimize these concepts.
