摘要: Transforming care for frail older adults requires more than rigorous research. While preventing, identifying and managing frailty are critical to reducing the personal health systems impact of worldwide, collaborative approaches research application that reflect stakeholder perspectives priorities necessary create meaningful solutions frailty-related challenges. In South Australia, a new Centre Research Excellence in Frailty was recently launched with funding from National Health Medical Council Australia. Comprised national team international partnerships expertise spanning geriatric medicine, nursing, general practice, economics, pharmacy rehabilitation is working across traditional disciplinary silos achieve system level improvements. Drawing this exemplar, we discuss how co-design approach knowledge translation underpins transdisciplinary research, successfully restructuring services meet physical, emotional social needs hinges upon such collaboration.
