摘要: A 47-year-old man was referred to treatment of a lesion located on the left maxilla with 5 months evolution. Intraoral examination revealed perforation in vestibular cortical bone. The computerized tomography (CT) showed an extensive, well-delineated, radiolucent lesion, extending from alveolar ridge up nasal cavity. Microscopically, cystic consistent odontogenic keratocyst (OKC) observed. Decompression posterior enucleation performed. After 3 months, patient returned presenting significant increase perforation. destructive lytic involving and surpassing medium line evident CT. second incisional biopsy presented as epithelial neoplasia composed islands projections toward surface abundant keratin deposition forming pearls plugging. Diagnosis primary intraosseous carcinoma arising OKC confirmed underwent maxillectomy. 1 year follow-up, remains without recurrence.
