Health-Related Quality of Life in Children and Adolescents after Open-Heart Surgery

摘要: Objective To assess health-related quality of life (HRQOL) in children with congenital heart disease (CHD) after open-heart surgery and to identify medical, individual, family-related predictors outcome. Study design Cross-sectional cohort study. 155 CHD (40% cyanotic type) who underwent between 1995 1998 were eligible. One hundred ten patients (response rate 71%) investigated at a mean age 10.4 years. Mean operation was 2.3 years (range, 0-8.7 years), duration cardiopulmonary bypass 95 minutes 5-206 minutes), circulatory arrest performed 9% the children. Child- parent-rated HRQOL evaluated by TNO-AZL Child Quality Life Questionnaire. Scores compared healthy references. Results Most dimensions self-reported impaired, including autonomy motor, social, emotional functioning. In addition children’s ratings, parents rated cognitive functioning their as diminished. Multivariate analyses revealed that bypass, length hospitalization, need for current cardiac medication, adverse family relationships had negative impact on parent- but not child-rated HRQOL. Conclusions is impaired surgery. Medical variables are important determinants.
