Clinical-computed tomographic correlations of lacunar infarction in the Stroke Data Bank.

摘要: Lacunar stroke was diagnosed in 337 (26%) of the 1,273 patients with cerebral infarction among 1,805 total Stroke Data Bank. We analyzed 316 classic lacunar syndromes. Among these, 181 (57%) had pure motor hemiparesis, 63 (20%) sensorimotor syndrome, 33 (10%) ataxic 21 (7%) sensory and 18 (6%) dysarthria-clumsy hand syndrome. No striking differences were found risk factors for subtypes, but between as a group other types infarcts. Compared to 113 large-vessel atherosclerotic infarction, those fewer previous transient ischemic attacks strokes. 246 cardioembolic more frequently hypertension diabetes less cardiac disease. lesion 35% patients' computed tomograms, most lesions located internal capsule corona radiata. The mean infarct volume greater hemiparesis or syndrome than subtypes. In infarcts posterior limb capsule, there correlation severity except few whose involved lowest portion capsule; these severe deficits occurred regardless volume. Taken together, tomographic correlations syndromes showed only slight support classical view homunculus capsule.
