摘要: Aims.  The purpose of this study was to develop a conceptual model discharge planning by nurses for clients returning home with high care needs. Background.  In Japan, plays an important role in the smooth and timely from medical facilities. Discharge often oversee such processes, especially needs. However, that guides during process is currently lacking. Design.  Qualitative. Methods.  A constant comparative approach used collect analyse data semi-structured interviews conducted 13 working at centres throughout Japan. Each nurse described her activities client needs. Results.  ‘Creating agreed discharge’ core category carried out nurses. consisted of: (1) developing blueprint, (2) reaching agreement, (3) materialising plan (4) sending home. starts blueprint client/family life after discharge. each thereafter their various activities. She or he also strives reach agreement regarding plan. detailed here are based on client/family’s daily life, adjustments made within agreeable range. Conclusions.  Developing revising refining it establish situations preferences coming further revisions final were essential process. precise projection client’s condition, community resource knowledge, estimating family understanding specific thoughts vital successful process. Relevance clinical practice.  These findings may be useful development guidelines.
