Two cases of severe de novo colitis in kidney transplant recipients after conversion to prolonged-release tacrolimus.

摘要: Diarrhea is a frequent complication in patients after solid organ transplantation. We describe two cases of severe new onset colitis kidney transplant recipients that developed shortly the introduction therapy with prolonged-release formulation tacrolimus replacing standard twice daily one case and cyclosporine A second case. Both severe, intermittent bloody diarrhea abdominal pain, weight loss, dehydration worsening graft function required immediate hospitalization. The symptoms did not diminish dose reduction or withdrawal mycophenolic acid derivatives. After excluding bacterial, viral, fungal, parasite infections, colonoscopy colonic biopsy was performed both patients, which revealed features typical colitis. received mesalazine until stopped. In immediately reintroduced. patient given everolimus an acute phase diarrhea. Although presented this report cannot fully support causal relationship between colitis, they should increase awareness among physicians prompt more close monitoring such potential side effects as part pharmacovigilance plan for well-established immunosuppressive drug.
