摘要: INTRODUCTION Despite improved treatment and access to care, adolescent AIDS deaths are decreasing more slowly than in any other age group. There is lack of longitudinal data around adherence the dynamics viraemia over time. We aimed describe patterns detectable viral load (VL) a cohort adolescents attending an ARV clinic Cape Town, South Africa. METHODS conducted retrospective study all patients on antiretroviral therapy aged 10 19 years. Participants were included if they underwent at least two VL measurements remained care Groote Schuur Hospital HIV Clinic for 24 months between 2002 2016. The primary outcome was consecutive >100 copies/mL, line with lower limit detection assays use follow-up period. RESULTS AND DISCUSSION Of 482 screened participants, 327 met inclusion criteria. Most participants had perinatally acquired (n = 314; 96%), 170 (52%) males. Overall, there 203 episodes confirmed involving 159 (49% (95% CI 43% 54%)) during Six genotyped resistance protease inhibitors. Four these never suppressed, while suppressed salvage regimens. Total time 1723 person years (PY), which 880 (51%) contributed by who experienced VL. Overall 370 PY. This comprised 22% total time, 42% those rate 11.8 10.3 13.5) per 100 risk increased 24% each year increasing (Relative Risk 1.24 1.17 1.31); p < 0.0001). no sex difference respect duration (p = 0.4), prevalence (p = 0.46) (p = 0.608) CONCLUSIONS Clinicians need be alert high adolescence so as pre-empt it act swiftly once diagnosed. helps highlight that associated increase well proportion poorly adherent spend this state.
