摘要: Abstract Background . There is still no perfect conduit for reconstruction of the right ventricular outflow tract (RVOT) in children. Homografts are not always available appropriate size, and degenerate a few years. This study evaluates pericardial valve with Gore-Tex as an alternative RVOT construction. Methods From January 1, 1993, to September 30, 1999, tissue was inserted all patients undergoing or pulmonary replacement (PVR) who were large enough accommodate valve. In without native main artery, new technique used construct RV-PA out flat sheet Gore-Tex, Dacron frequently leads stenosis. Data collected by retrospective review, follow-up echocardiograms, assessment single cardiologist. Results 48 patients, 22 PVR alone 26 valved conduit. Diagnosis included tetralogy Fallot (n = 25); truncus arteriosis 9); septal defect PA 5); DORV 4); D-TGA PS 2); 1 each IAA sub AS, VSD PI, s/p Ross procedure. Patient age ranged from 3 33 years 98% reoperations. The sizes 19 mm median hospital length stay 4 days. 2 (4.2%) perioperative (2.1%) late deaths, none related At 15 86 months (mean 43 ± 16 months), remaining 45 New York Heart Association class I, valves functional, patient has required revision; more importantly, echocardiogram revealed significant stenosis gradient 8 Hg) evidence regurgitation structural degeneration. Conclusions A provides reliable pediatric patients. It readily available, molds limited retrosternal space, outstanding intermediate results failure deterioration up 7 after insertion.
