摘要: Objectives: Industrial trans fatty acids (TFA) increase CHD risk, but use remains high in developing nations. From 2004-2014, Argentina initiated policies to reduce industrial TFA: voluntary agreements with industry 2004, mandatory labeling 2006, and elimination by Food Code 2014. We aimed evaluate the policy’s impact on CHD, DALYs costs. Methods: A review of diet trials, local studies, consultations experts was performed estimate baseline TFA intake 2004 types replacements used from 2004-2014. built an epidemiological simulation model calculating 3 scenarios effects isocaloric replacement alternative fats/oils, based (1) changes total:HDL-C ratio (Δ TC/HDL) (2) other risk biomarkers, predicted these biomarker CHD; (3) observed associations which events prospective may better account for pleiotrophic effects. Framingham equations first vs. without policy according individual age-sex-risk factor distributions (including ΔTC/HDL), utilizing Argentine population-based sample 4,000 adults weighted full population. Results: After 2014, this would prevent each year 1.3% (based Δ TC/HDL only; 7.4% effects) all (Table). Accounting intervention costs averted events, is actually cost-saving. Conclusions: This evaluating actual eliminating a country. Given 165,000 annual Argentina, at incidence rate almost 1% adults, near might avert under most conservative assumptions, saved $36 million. In low-resource settings, where higher, could be larger. Our findings inform makers countries huge
