摘要: Skull base approaches play a fundamental role in modern neurosurgery by reducing surgical morbidity. Increasing experience has allowed surgeons to perform minimally invasive without straying from the premises of skull surgery. The eyelid approach evolved orbitopterional osteotomy into more effective and targeted disease anterior cranial fossa. In this technique, after an incision is made on supratarsal fold, orbicularis oculi muscle incised, myocutaneous flap composed elements lamella elevated. Subperiosteal dissection used expose superior lateral walls orbit, orbital rim, frontosphenoidal suture. A MacCarty bur hole drilled, frontal fashioned medial supraorbital notch extending through roof back toward half hole, exposing frontobasal brain. conventional microsurgical technique treat tumors aneurysms fossa under operative microscope. Five patients were treated for unruptured circulation (3 communicating artery aneurysms, 1 ophthalmic aneurysm, posterior aneurysm) using approach. mean aneurysm size was 5 mm, all approached right side. Three (2 suprasellar pituitary adenomas craniopharyngioma) also excised There no months surgery presented excellent cosmetic results. may be considered as effective, cosmetically beneficial, selected circulation.
