The conservative management of primary pyomyositis abscess in children: case series and review of the literature:

摘要: Pyomyositis is a primary pyogenic infection in skeletal muscle, often progressing to abscess formation. It rare temperate climates and generally deep-seated within the pelvis with non-specific clinical features, making diagnosis difficult. Magnetic resonance imaging (MRI) highly sensitive for muscle inflammation fluid collection, its increasing availability now investigation of choice. Treatment pyomyositis has traditionally been incision drainage or guided aspiration followed by prolonged course antibiotics, although there are sporadic reports cases treated successfully antibiotics alone. Our aim was describe our own experience treatment children. From 20-year database over 16,000 paediatric orthopaedic admissions, we identified only three MRI-confirmed abscess. These were all boys (aged 2-12 years) affected gluteal, piriformis adductor muscles. Despite organisms not being identified, each patient short (4-7 days) intravenous 2-6 weeks oral therapy. There no recurrences complications made full recovery. In conclusion, propose that uncomplicated children may usually be managed conservatively without need open percutaneous drainage.
