摘要: The Xpert MTB/RIF assay is an automated molecular test that designed to simultaneously detect Mycobacterium tuberculosis (MTB) complex and rifampin resistance. However, there are relatively few studies on this method in China. has been routinely used at Peking University People’s Hospital (PKUPH) since November 2016. Thus, the aim of study was evaluate performance Xpert, provide a reference guidance for detection diagnosis TB non-TB specialized hospitals. medical records inpatients tested with acid-fast bacilli (AFB) smear microscopy, interferon-gamma release (IGRA, by T-SPOT®.TB) PKUPH from 2016 October 2018 were reviewed. Active cases considered according composite standard (CRS). Then, three methods evaluated compared. In total, 787 patients AFB, IGRA enrolled; among them 11.3% (89/787) diagnosed confirmed active pulmonary (PTB, 52 cases), extrapulmonary (EPTB, 17 tuberculous pleurisy (TP, 20 cases). sensitivity detecting PTB, EPTB, TP 88.5, 76.5, 15.0%, respectively, which slightly lower than (96.2, 82.4, 95.0%, respectively), but higher AFB (36.5, 11.8, 0%, respectively); showed highest sensitivity, its specificity (55.9, 67.1, 45.2%, respectively) significantly (99.6, 99.4, 100%, (99.0, (P < 0.001). lung tissue, cerebrospinal fluid, lymph nodes, joint fluid followed sputum (88.5%), alveolar lavage (85.7%), bronchoscopy secretion (81.2%); pleural lowest, only 15.0%. For negative patients, 84.9, 73.3, respectively. both high specificity, suggested value diagnosis; however, application still limited, should be improved. Owing IGRA, it recommended use as supplementary test, especially assisting EPTB.
