摘要: Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is the standard surgical procedure for ulcerative colitis (UC) and familial adenomatous polyposis (FAP). While minimal invasive techniques have been applied increasingly, clear evidence of superiority laparoscopic pouch procedures not yet available. The aim LapConPouch Trial was to compare effectiveness (LAP) versus conventional (CON) ileoanal in patients undergoing elective restorative proctocolectomy. trial designed as a single-centre, pre-operatively randomized, controlled using two-group parallel design. Eligible participation were scheduled either FAP or UC. Patients outcome assessors blinded group assignment. primary endpoint defined amount blood loss. Statistical analyses explorative since had be stopped prematurely. A total 42 (21 LAP (50.0 %); 21 CON (50.0 %)) randomized. prematurely due insufficient patient recruitment. There no difference loss between both groups: 261.5 ± 195.4 ml, 228.1 ± 119.5 ml. Secondary endpoints differ groups. Laparoscopic surgery superior regarding length skin incision; contrast, approach duration operation. discrepancies hospital stay, postoperative pain, bowel function, quality life approaches. conversion rate from 23.8 %. respect group. feasible proctocolectomy, IPAA seems at least safe surgery. most obvious advantage technique improved cosmesis.
