摘要: Background: Intra-operative PTH testing in the operating theatre has proven to be an accurate way verify removal of all pathological parathyroid tissue primary hyperparathyroidism. Its limitation is high cost. An alternative, more cost-effective procedure proposed: intra-operative dosage at Central Laboratory. Patients and methods: Fifty-four patients underwent parathyroidectomy with intraoperative Three blood samples were taken from each patient: just after induction anesthesia, 5 10 min parathyroidectomy. The sent Laboratory analysed simultaneously. results phoned back theatre. was considered effective when drop ≥50% basal value, Results: 98.1% proved recovered (average follow-up 31.1 months). had 3 false negatives, 1 positive, sensitivity, specificity, accuracy, positive predictive value negative 94.0%, 75.0%, 92.6%, 97.9%, 50.0%, respectively. Discussion conclusion: main disadvantage presented long waiting time. Nevertheless this time same as that required for histological examination, only alternative determine surgery effectiveness centres where portable instrumentation not available. advantage very low If terms cost are into consideration, may deemed a viable
