Activation of the alternative pathway complement is a feature pre-term parturition but not spontaneous labor at term. Am J Reprod Immunol 2010; 63: 318–330 Problem  Plasma concentrations fragment Bb (FBb) are marker for activation system. High FBb in maternal blood, as early first trimester, associated with subsequent delivery <34 weeks gestation. The aim this study was to determine whether (PTL) intact membranes, intra-amniotic infection/inflammation (IAI) or term alterations circulating concentrations. Method study  This cross-sectional included women following groups: (i) non-pregnant (n = 40); (ii) normal pregnancy (gestational age range 20–36, 6/7 weeks, n = 63); (iii) (n = 70); (iv) (n = 59); (v) patients an episode PTL who delivered (n = 62); (vi) without IAI (n = 30); and (vii) (n = 67). Maternal plasma were determined by ELISA. Results  Among PTL, those had either (1.21 μg/mL, IQR 0.77–2.16) (1.13 μg/mL, 0.92–2.08) higher median concentration than (0.86 μg/mL, 0.64–1.57; P = 0.007 P = 0.026, respectively); there no difference between (P = 0.9); contrast, significant change (P = 0.8); did differ significantly pregnant controls (P = 0.8) correlated advancing gestational (r = −0.28, P = 0.8). Conclusion  Pre-term circulation; such detectable term; does explain PTL. Collectively, these observations suggest that labors have fundamental differences regulation innate immunity.
