摘要: The primary purpose of the present analysis was to assess feasibility and acute toxicity a pure accelerated fractionation regimen in cooperative group setting. This included first 320 patients entered on Trans-Tasman Radiation Oncology Group (TROG) randomized controlled trial which compared radiotherapy (ART) with conventional (CRT) stage III IV squamous cell carcinoma (SCC) head neck. Patients were either 59.4 Gy 33 fractions over 24 days or 70 35 49 after being stratified for site stage. Accrual began 1991 closed 3 April 1998 targeted 350 patients. 3-year survival whole 54%, disease-free 41%. Toxicity data available 303 (148 ART; 155 CRT). Mucosal worse arm, it peaked approximately weeks earlier than arm. Skin equivalent but occurred 7 Acute effects both arms healed completely. Hospitalization more common ART arm (71 vs 52 patients; P = 0.01) total bed hospital not greatly different (1707 1607 likely require nasogastric (NG) feeding (49 0.02). There 1157 NG 1154 CRT. average cost radiation treatment per patient including hospitalization, accommodation $11,750 $11,587 CRT has been shown be tolerable, practical cost-equivalent regimen. assessment therapeutic ratio this protocol will determined when late loco-regional control is made are mature.
