Interventions to optimize pharmacologic treatment in hospitalized older adults: A systematic review

摘要: Abstract Objective To summarise the evidence on interventions aimed at optimising drug treatment of hospitalised elderly patients. Material and methods We conducted a search in main medical literature databases, selecting prospective studies patients older than 65 years who underwent treatment, decreasing polypharmacy improving medication appropriateness, health outcomes exploitation healthcare system. Results selected 18 whose consisted reviews, detection predefined drugs as potentially inappropriate for elderly, counselling from specialised geriatric team, use computer support system prescriptions specific training nursing team. Up to 14 assessed 13 which showed an improvement one or more parameters. Seven measured impact intervention polypharmacy, but only improved compared with control. other analysed mortality, none them reduction that rate. Only 1 6 number hospital readmissions, 4 emergency department visits. Conclusions Despite heterogeneity variables, we obtained better results process variables (especially appropriateness) those outcomes, had greater variability.
