摘要: Objective: To evaluate the predictive value of total serum bilirubin (TSB) ≤6 mg/dl at 24±6 hr postnatal age in identifying near term and infants, who do not develop hyperbilirubinemia subsequently. Design: Prospective study. Setting: Tertiary care hospital. Methods: All healthy neonates with gestation ≥ 35 weeks, absence significant illness or Rh hemolysis were included. TSB was estimated by micromethod using spectrophotometry. Infants followed up clinically every 12 till discharge then after 48 hr. level again whenever clinical suspicion jaundice exceeded 10 mg/dl. Primary outcome defined as presence (TSB 17 mg/dl) day five age. Results: Of 220 213 (96.8%) up. infants exclusively breastfed. Mean estimation 24. 7 ± 1.9 mean 5.9 1.8 Clinically detectable present 164 (77%) occurred 22 (10.3%) infants. A ≤ 6 24 136 (63.8%) only one infant developed subsequently (probability mg/dl, subsequent 21 (27.2%) (sensitivity 95%. specificity 70.6%, positive 27.2%, negative 99.3%, likelihood ratio test 3.23 0. 07). Babies levels higher than had a risk developing (relative 38; 95% confidence interval 6-1675). Conclusion: 6mg/dl life predicted would hyperbilirubinemia.
