摘要: Summary Background Severe cardiac glycoside cardiotoxicity after ingestion of yellow oleander seeds is an important problem in rural areas Sri Lanka. Currently, patients must be transferred to the capital for temporary pacing. We did a randomised controlled trial investigate whether antidigoxin Fab could reverse serious oleander-induced arrhythmias. Methods After preliminary dose-finding study, 66 who presented hospital with arrhythmia were receive either 1200 mg anti-digoxin or saline placebo. A 12-lead electrocardiogram, 3 min rhythm strip, and blood sample measurement electrolytes glycosides taken before treatment at 12 timepoints thereafter. Findings 34 received 32 The presenting had resolved completely 2 h 15 antibody-treated two controls (p Interpretation Anti-digoxin fragments are safe effective arrhythmias induced by oleander. Their use small hospitals Lanka should minimise costly transfer reduce numbers deaths; however, further study will required confirm this reduction.
