摘要: Abstract With 1-in-4 older adults suffering a fall each year, prevention efforts have emerged as public health priority. Multi-level, evidence-based programs been promoted by the CDC and other government agencies. To ensure participants communities receive programs’ intended benefits, organizations must repeatedly deliver over time plan for program sustainability part of ‘scaling up’ initiative. The State Falls Prevention Project (SFPP) began in 2011 when provided 5 years funding to Departments Health Colorado, New York, Oregon simultaneously implement four strategies: 1) Tai Chi: Moving Better Balance; 2) Stepping On; 3) Otago Exercise Program; 4) STEADI (STopping Elderly Accidents, Deaths, Injuries) toolkit. Surveys were performed examine systems change perceptions about across states. purposes this study to: how influenced capacity implementation within SFPP; assess reported Program Sustainability Assessment Tool (PSAT) scores learn best sustain preventing after ends. Data showed that more offered participants’ service areas with funding, importance programming implementation, evaluation, reporting likely diminish once concluded. Participants’ PSAT perceived did not directly align previously domain or modifiability. Findings suggest grantees identify potential barriers enablers influencing during planning phase programs.
