摘要: Background Endovascular repair of thoracic aortic aneurysms is an attractive alternative to surgical graft replacement, but many patients are excluded because unsuitable proximal landing zones wider than 38 mm in diameter. Arch can be repaired with less invasive hybrid surgery, combining endovascular stent grafting banding the aorta. Methods and Results Since November 2006, 6 88 underwent arch presumptive zone 0 as site where median maximal diameter was 42.5 (from 39 44 mm). After partial sternotomy, relocation supra-aortic branches, ascending aorta banded a 2-layer Dacron strip reduced 36±1 inserted incorporated securely. All were uncomplicated, complete exclusion lesion without endoleaks or migration during average 9 months' follow-up. Conclusions Secure fixation solved via will not only improve short-term stability intervention, may become preferred therapeutic modality multiple comorbidities. (Circ J 2008; 72: 1981 - 1985)
