摘要: While disease management appears to be effective in selected, small groups of CHF patients from randomized controlled trials, its effectiveness a broader patient population is not known. This prospective, quasi-experimental study compared outcomes under nurse practitioner-led model (intervention group) with usual care (control both primary and tertiary medical centers. The included 969 veterans (458 intervention, 511 control) treated for at six VA Intervention had significantly fewer (p<0.05) all-cause admissions one-year follow-up, lower mortality one- two-year follow-up. These data provide support the potential suggest that evidence RCTs models can translated into clinical practice, even without benefits selected dedicated resources often found RCTs.
