Direct Neck Clipping for Unruptured Basilar Tip Aneurysm via Transsylvian Approach

摘要: Surgery of direct neck clipping basilar tip aneurysms is one the most challenging procedures in vascular neurosurgery, as these lesions are deeply situated interpeduncular region and maintain an intimate relationship with important anatomical structures. So this location now frequently treated endovascular technique. Microsurgical occlusion technique still, however, maintains its solid position because completeness. We suggest transsylvian approach for location. There several tactics to improve operative results. A wider surgical field can be obtained by dissecting sylvian fissure from distal segment. And anterior clinoidectomy unroofing optic canal widen space around internal carotid artery cisternal cavity. Transection posterior communicating needed particular cases. The temporary method intensive hypotension useful perforating arteries ensuring complete clipping.
