摘要: Objectives: Elective neck dissection for the N0 in head and surgery is still controversial. This prospective nonrandomized study of supraglottic carcinoma was designed to find an appropriate method management. Study Design: Anatomical studies show that first echelon lymphatic drainage from larynx toward upper jugular nodes (level II). An (UND) applied all lymph were sent frozen section. If subclinical metastasis found, a modified performed. harbored no foci cancer, patients observed after on lesions. Methods: Patient records 142 with laryngeal cancer (T1–4N0M0) reviewed, special attention paid recurrences survival rates. The cases treated between 1976 1990 at least 5 years operation or until time death. Results: UND specimens negative metastasis. 5-year rate this group 80.8%, according life table analysis. Fifteen (10.6%) had during period observation within years. recurrence series limited comparable those reported literature dissection, either radical modified. Conclusions: There need comprehensive cancer. A selective such as II) supraomohyoid (sparing submandibular region) level II III will serve purpose treatment
