摘要: The New York University Medical Center Cooperative Care (CC) program is a model of delivery system acute inpatient hospital care characterized by live-in family member or friend acting as "care partner". It has an emphasis on education in order to encourage full patient and involvement during the hospitalization, thereby preparing both parties for management at home after discharge. education-intensive experience CC provides alternative traditional with expected outcome being increase knowledge satisfaction, adherence medical regimen, appropriate self-management. functioning ability patient-care partner team should be improved discharge, which may result decreased subsequent utilization high cost healthcare resources such rehospitalization.
