Total parenteral nutrition as the sole therapy in Crohn's disease--a prospective study.

摘要: The effect of total parenteral nutrition (TPN) as sole therapy was studied in 30 consecutive cases complicated Crohn's disease. After insertion a Broviac-type central venous catheter patients were nourished parenterally for 3 weeks the hospital and then an additional 9 at home. During this time no medication or oral intake allowed. Surgery avoided 25 by TPN. These returned to work, ate normal meals needed medical support. In 5 it not possible control acute disease treated resection. TPN, catheter-related sepsis occurred (0.9 cases/1000 days TPN) embolism 2. Four other developed intrahepatic cholestasis. A relapse observed 17 3-48 months after course cumulative recurrence rate is 60 per cent 2 years 85 4 years. Compared with results resection, obtained from 10-year period before TPN instituted our hospital, four times higher. It concluded that alternative resection treatment should be reserved multifocal lesions, when surgery advisable because risk short bowel syndrome.
