Prostaglandins: A New Approach to Glaucoma Management with a New, Intriguing Side Effect

摘要: Abstract This introductory overview considers the advantages of a class local hormones — prostaglandins (PGs) for management intraocular pressure (IOP) in glaucoma, over agonists and antagonists neurotransmitters that dominated this field 20th century. PGs PG analogues, particular esterified prodrug forms PGF2α, are effective ocular hypotensive agents, but cause some conjunctival hyperemia corneal sensory irritation at higher concentrations. Based on structure-activity studies, 17-phenyl prodrug, latanoprost (PhXA41), was found to have greatly improved therapeutic index, without compromising potency PGFz, prodrugs. The IOP lowering mechanism such PGF2αs, increased uveoscleral outflow, can be expected great physiologic advantages, especially with respect normal tension most currently used drugs. introduction new approach has already led insight into control clinical significance outflow route. Similarly, newly discovered side effect, PG-induced increase iridial pigmentation, provide oculo-protective role ofiridial melanocytes punative association between decline melanin system vulnerability eye age-related diseases.
