PHARMACOCINETIQUE ET SUIVI THERAPEUTIQUE PHARMACOLOGIQUE DU MYCOPHENOLATE MOFETIL DANS LE TRAITEMENT ANTI-REJET DE GREFFE

摘要: Le but de ce travail etait d’etudier la pharmacocinetique (PK) l’acide mycophenolique (AMP, metabolite actif du mycophenolate mofetil, Cellcept ®) chez des patients transplantes renaux afin developper outils d’ajustement individuel posologie mofetil. Une methode chromatographie liquide - spectrometrie masse en tandem (LC-MS/MS), specifique et sensible, a ete developpee validee pour le dosage l’AMP son principal l’AMP-phenyl-glucuronide. L’analyse non-compartimentale mis evidence une grande diversite profils concentration individuels obtenus lors d’essais cliniques novo ou stables, l’effet delai post-transplantation sur leur evolution. L’utilisation d’une deux distribution(s) Gamma decrire phase d’absorption medicament permis, grâce au logiciel MMF ® developpe dans l’equipe (Dr. Jean Debord), modelisation efficace ces complexes d’AMP, presentant frequemment un deuxieme pic d’intensite tres variable. etude PK population egalement realisee par approche parametrique etape (NONMEM) quantifier les variabilites inter- intra- individuelles d’identifier covariables pouvant expliquer partie variabilites. Sur base modeles developpes avec ®, nous avons point valide estimateurs bayesiens permettant predire parametres moyen trois prelevements sanguins. En parallele, equations estimant l’AUC 0-12h partir d’un nombre limite concentrations sanguines ont elaborees regression lineaire multiple. Ces travaux fourni necessaires Suivi Therapeutique Pharmacologique prevention rejet aigu greffe renale, actuellement utilises clinique multicentrique.
