Abstract:  Background:  Patients transplant waiting lists continue far exceed donor rates. We seek understand the barriers preventing people city from donating organs for transplantation. Methods:  One thousand adults were surveyed assessing knowledge, personal involvement, attitudes about donation urban center Canada. Primary outcomes of interest signed card willingness donate. Results:  Of those surveyed, 64% did not realize that they possessed card; 90% would consider being if friend was need organ. 36% who know, 72% had it. Those misconceptions process less likely be donors. Interpretation:  There is tremendous lack knowledge donation. While majority are interested donation, means express this – most do even possess card. A significant proportion supportive misinformed critical areas influenced decision including rich preferentially transplanted, consent process, disfigurement, donors receiving worse medical care.
