摘要: Hypothesis Live donor adult liver transplantation (LDALT) is a safe and efficacious treatment for patients with end-stage disease. Design Case-control study. Setting Hepatobiliary surgery unit. Patients From December 10, 1998, through April 2000, single team performed 15 LDALT procedures 2 simultaneous living kidney transplants. During this period, 66 potential donors were screened evaluated. Interventions Potential evaluated 3-dimensional helical computed tomographic scan, including volume renderings hepatic lobar volume, vascular anatomy, virtual resection planes, morphologic features. Suitable undergo complete medical psychiatric evaluation preoperative arteriography. Main Outcome Measures Donor demographics, data, operative hospital length of stay, morbidity. Results A total 38 men (58%) 28 women (42%) participating in LDALT. Two additional provided grafts at the time Thirty-two (48%) rejected either or recipient reasons, 10 (15%) elected not to participate after initial screening. Three-dimensional by scan predicted right lobe within 92% actual graft volume. morbidity, all complications, was 67% no mortality. Residual regenerated approximately 70% 1 week 80% month surgery. Conclusions an important component Significant morbidity encountered even careful selection. To minimize groups considering initiating programs should have expertise vena cava preservation using "piggyback" technique during transplantation.
