Hashimoto's encephalopathy: epidemiologic data and pathogenetic considerations

摘要: Background: Hashimoto's encephalopathy (HE) is a condition believed to complicate thyroiditis (HT). The diagnosis suspected in the presence of high levels serum anti-thyroid antibodies. We have recently demonstrated that patients with HE there an intrathecal synthesis antibodies, and concluded should be based on this cerebrospinal fluid (CSF) finding. Objective: getting estimate prevalence disease, verifying association HT investigating pathogenetic role Methods: 34-months prospective study hospital setting serving community 150,000 people. Patients unexplained symptoms acute or subacute myelopathy history thyroid disorders were selected for measurement In autoantibodies, same tests performed CSF. Results: Twelve had increased concentrations autoantibodies but was diagnosed only nine patients. estimated 2.1/100,000. Only six also HT. Four received corticosteroids, five not treated. Five improved, four spontaneously, one patient after corticosteroids. Repeated CSF examinations showed titer did correlate clinical stage disease nor influenced by addition, course independent therapy. Conclusions: titers antibodies sufficient make HE. Independent status gland, distinctive marker elusive condition.
