Arthroscopic foveal repair of the triangular fibrocartilage complex.

摘要: Background  Foveal disruption of the triangular fibrocartilage complex (TFCC) is associated with distal radioulnar joint (DRUJ) instability. TFCC fixation onto fovea suitable treatment, which not achieved by conventional arthroscopic techniques. We describe an all-inside technique that uses a suture anchor through DRUJ arthroscopy for foveal repair TFCC. Materials and Methods  Forty-eight patients tear instability were selected according to Atzei–European Wrist Arthroscopy Society (EWAS) algorithm treatment. Retrospective evaluation included pain, instability, range motion (ROM), grip strength, Modified Mayo Score (MMWS), Disabilities Arm, Shoulder, Hand (DASH) Score. Description Technique  DRUJ was performed debride area. Under guidance, inserted via portal fovea. Sutures tied on radiocarpal surface Postoperative immobilization forearm rotation maintained 4 weeks. Heavy tasks allowed after 3 months. Results  After mean follow-up 33 months, pain improved significantly but remained moderate in four patients, severe one. resolved 44 patients. ROM increased. Grip MMWS, DASH score significantly. Excellent good MMWS equaled 83.3%. Forty-one (85.5%) resumed previous work sport activities. As postoperative complication, five experienced neuroapraxia dorsal sensory branch ulnar nerve (DSBUN) full spontaneous recovery. Conclusions  With appropriate indications patient selection, may restore stability provide satisfactory results without significant complications.
