摘要: Purpose To identify barriers to safe and effective completion of outpatient parenteral antimicrobial therapy (OPAT) in patients discharged from an academic medical center develop targeted solutions potentially resolve or improve the identified barriers. Summary A failure modes effects analysis (FMEA) was conducted by a multidisciplinary OPAT task force evaluate processes for on 2 postdischarge dispositions: (1) home (2) skilled nursing facility (SNF). The created process maps potential modes, barriers, successful each step. Thirteen 10 were SNF maps, respectively. Task members 5 subgroups, developing group related areas focus included electronic order set, critical tasks be performed before patient discharge, (3) education, (4) follow-up laboratory monitoring, (5) communication. Interventions involved working with information technology update bridging communication ensuring creating inpatient discharge checklist, education resources, planning central database within record, pharmacist on-call pager SNFs. Conclusion FMEA approach helpful identifying perceived transitions care interventions. Healthcare organizations may reproduce this strategy when completing quality improvement high-risk process.
