摘要: Abstract Objectives To evaluate community pharmacists’ reimbursed prescribing interventions (called pharmaceutical opinions/POs in Canada) by determining the types of drug-related problems (DRPs) identified; type, quality, and clinical impact recommendations made; variation recommendation quality across regions, pharmacy ownership size. Design Retrospective audit randomly sampled service documentation records. Setting Community pharmacies Ontario, Canada, 3 years after implementation a government program that reimburses for providing DRP-based POs to physicians. Participants Thirty-six 4 regions Ontario. Main outcome measures For each PO, 2 experienced pharmacists independently, then consensus, determined primary drug implicated its therapeutic category, type DRP, prescriber recommendation. Each PO was assessed impact, evidence base, overall quality. Chi-square analyses relationship between characteristics. Results Of 563 POs, 261 (46%) met reimbursement criteria stating DRP Among these eligible systemic antiinfective (22%), alimentary tract metabolism (19%), nervous system (15%), cardiovascular (15%) drugs were most commonly identified DRP. Adverse reactions (which included interactions) common (42.5%), followed “dose too high” (16%) “needs additional therapy” (14%). Top change (41%) decrease dose (20%).
