Employment of telemedicine in emergency medicine. Clinical requirement analysis, system development and first test results.

摘要: Objectives: Demographic change, rising co-morbidity and an increasing number of emergencies are the main challenges that emergency medical services (EMS) in several countries worldwide facing. In order to improve quality EMS, highly trained personnel well-equipped ambulances essential. However studies have shown a deficiency qualified EMS physicians. Telemedicine emerges as complementary system may provide expertise treatment on scene. Hence our aim is develop test specific teleconsultation system. Methods: During development process use cases were defined technically specified by experts engineers areas of: administration, start-up assistance systems, audio communication, data transfer, routine tele-EMS physician activities research capabilities. Upon completion, technical field tests performed under realistic conditions properties such robustness, feasibility usability, providing end-to-end measurements. Results: Six equipped with telemedical facilities based results requirement analysis 55 scenarios tested one month. The indicate developed well terms usability robustness. major were, expected, mobile communication network availability. Third generation networks only available 76.4% cases. Although 3G (third generation), Universal Mobile Telecommunications System (UMTS), provides beneficial for higher bandwidth, performance most features was also acceptable adequate 2G (second generation) conditions. Conclusions: An innovative concept telemedicine consultations developed. Organisational aspects considered practical requirements specified. Since demonstrated these tests, next step would be prove usefulness robustness real clinical trial.
