Pneumothorax during laparoscopic Nissen fundoplication.

摘要: We present a case of 100% pneumothorax in 41-yr-old man with history gastritis and gastroesophageal reflux scheduled for Nissen fundoplication. The patient was anaesthetized, insufflation the abdominal cavity carbon dioxide performed uneventfully. There an increase peak inspiratory pressure wheezing noted decrease arterial oxygen saturation to 91%. An obstructive pattern on end tidal monitor. also had decreased breath sounds left lung field. endotracheal tube withdrawn 1.5 cm equal both fields, but persisted. At trocars were removed abdomen deflated. increased 94% while breathing FIO2 1.0. A chest roentgenogram showed pneumothorax. placed immediate improvement 100%. recommend monitoring saturation, pressures, excursion early diagnosis prompt treatment during laparoscopic procedures.
