Laparoscopic duodenum-preserving total pancreatic head resection: a novel surgical approach for benign or low-grade malignant tumors.

摘要: Duodenum-preserving total pancreatic head resection (DPPHRt) is an accepted alternative surgical procedure for benign or low-grade malignant tumors of the by preserving duodenum with its intact blood supply from duodenal arterial arcade. This study describes our experience in laparoscopic DPPHRt (LDPPHRt). To knowledge, this first description novel minimally invasive operation. From August 2016 to May 2017, all consecutive patients who underwent LDPPHRt lesions at HPB Surgery Department, Sun Yat-Sen Memorial Hospital Guangzhou, China were enrolled into retrospective study. There ten women and two men. The average age was 37.3 years (range 8–61 years). diameter on pre-operative CT/MR 3.7 cm 2–4.8 cm). All procedures performed successfully. no peri-operative death. operative time 272.5 min 210–320 min). loss 215 ml 50–450 ml). Post-operative complications included fistula grade B (two patients, 16.7%) biliary 16.7%). responded well conservative treatment. mean post-operative hospital stay 11.5 days 6–25 days). provided a approach good organ-preservation head. long-term oncological outcomes, exocrine endocrine functions after operation require further studies.
