Neurological and neuropathological sequelae of correction of chronic hyponatremia

摘要: The effect of correction chronic hyponatremia at different rates was studied in 91 rats maintained a plasma [Na+] 112 +/- 1 mmol/liter for 19 days. Hyponatremia corrected into normal ranges (140 to 145 mmol/liter) using three methods. Rats by water restriction achieved 2.1 0.2 day and had maximal (4 hr) rate 1.0 0.1 mmol/liter.hr; diuresis 1.6 2.8 hypertonic saline infusion 5.4 0.3 hr 5.7 0.4 mmol/liter.hr. A fourth control group not corrected. No demyelinative lesions were found the brains from uncorrected rats, whereas occurrence such highly correlated with increase (r = 0.68, P less than 0.001), lesser degree magnitude over first 24 hours 0.41, 0.001). Brain myelinolysis observed animals whose exceeded 1.75 mmol/liter.hr, incidence demyelination increased progressively more rapid correction.
