摘要: The goal of community-based services for frail older patients is to help them achieve the greatest degree functional ability and independence. available include case management, geriatric assessment, adult day health care, home services, Program All-inclusive Care Elderly (PACE). Definitive criteria referral have not been established, but without some targeting, efficacy these remains uncertain. Targeting identified dependency in 2 or more activities daily living, no family support, dementia, many long-term illnesses, hospital stays. Although cost-effectiveness remain uncertain, patients, families, physicians generally report be helpful.
