Impact of Human Immunodeficiency Virus Infection on Progression to End-Stage Liver Disease in Individuals with Hemophilia and Hepatitis C Virus Infection

摘要: Hepatitis C virus (HCV) is the major cause of chronic liver disease in hemophiliacs. To determine effect human immunodeficiency (HIV) on natural history HCV infection, we evaluated end-stage (ESLD) 157 hemophiliacs (85 HIV positive and 72 negative) with infection for an average 24 years. After adjusting age at past or current hepatitis B surface antigen positivity, alcohol abuse, determined that rate ESLD was significantly greater among HIV-positive than HIV-negative (relative risk [RR], 3.72; 95% confidence interval [CI], 1.25-11.09), as adjusted RR death due to (RR, 3.81; CI, 1.19-12.16). Among hemophiliacs, crude lower, but not so, antiretroviral treatment 0.19; 0.03-1.14; P = .069) increased each decade 2.26; 1.42-3.59; .0006) 2.18; 1.36-3.49; .0013). These findings suggest accelerates progression.
