摘要: The investigation is based on the construction of a new health model under strategies Primary Health Care (APS), which was applied in year 2004 to 199 population chronic patients users Colombia`s National Police system survey and clinical review before after intervention. Very interesting results were obtained contributes with knowledge for country especially Military Forces Police.
