摘要: Mechanical ventilation is an essential method of patient support, but it may induce lung damage, leading to ventilator-induced injury (VILI). VILI the result a complex interplay among various mechanical forces that act on structures, such as type I and II epithelial cells, endothelial macrophages, peripheral airways, extracellular matrix (ECM), during ventilation. This article discusses ongoing research focusing mechanisms in previously healthy lungs, perioperative period, development new ventilator strategies for surgical patients. Several experimental clinical studies have been conducted evaluate mechanotransduction each cell ECM, well role different parameters inducing or preventing VILI. be attenuated by reducing tidal volume; however, use higher lower levels positive end-expiratory pressure (PEEP) recruitment maneuvers period matter debate. Many questions concerning patients remain unanswered. The optimal threshold value parameter reduce also unclear. Further are necessary better settings types surgery.
