Readmission after Open Heart Surgery: Study of Predictors and Frequency

摘要: Background: Hospital readmissions after cardiac procedures are believed to be associated with higher in-hospital mortality and may predict poor outcomes. In addition high rate of readmission following discharge is increased cost care. Therefore, awareness factors that risk for hospital surgery improve the ability reduce early rates among this category patients. Aim study; assess predictors surgery. Design: A descriptive exploratory design was utilized in current study. Subjects: sample Convenience including 115 adult male & female patients who were admitted cardiothoracic departments at Kasr Al-Aini Hospital, Cairo University over a period six months recruited. Tools data collection: Four tools collect pertinent study: Socio-demographic/medical sheet; Perioperative open heart assessment Sheet; LACE index Scale (Lengths stay days; Acuity illness time admission; Carlson co-morbidity score; Emergency department visits numbers during previous months. Results: study revealed majority studied males, married, having an elective admission percentages (67.8%), (86.1%) (87%) respectively, nearly half them between 40–59 years came from rural area (50%) (52.2%) respectively. Out discharged 18 % required second ICU readmission. Main reasons wound problems (42.9%), congestive failure (14.3%), atrial fibrillation (9.5%), pleural effusion renal (9.5%) respiratory (4.8%). Binary logistic regression analysis preoperative failure, delayed extubation (mechanical ventilation > 8 h), re-exploration bleeding, perioperative use intraaortic ballon pump (IABP), postoperative dysrhythmias, postsurgical (ICU) length (> 3 day) independent Conclusion Recommendations: Based on findings study, it can concluded important adverse outcome needs continued attempts explore manage So recommends identification close monitoring those readmission; Establishment hot line services providing proper consultation discharge, especially emergent cases; provision surveillance units detection Keywords: Open Heart Surgery- Readmission-Predictors- Frequency.
