Course and outcome of chronic pancreatitis. Longitudinal study of a mixed medical-surgical series of 245 patients.

摘要: Over the last 20 yr, 245 patients with chronic pancreatitis (163 alcoholic relapsing pancreatitis; 145 of them calcific pancreatitis) were prospectively studied at regular intervals particular regard to pain, pancreatic functions, calcifications, surgery, and survival. The median period observation in group was 10.4 yr. In this patients, 85% experienced lasting pain relief within a time 4.5 yr from onset. A gradual increase calcifications dysfunction observed increasing duration disease. Pain accompanied by marked calcification. Of 163 pancreatitis, 87 (53%) needed no surgery. Seventy-six (47%) recurrent or persistent severe mainly due pseudocysts (n = 56), underwent either cyst drainage procedure 22), papillotomy 4), distal pancreatectomy (40%-60%, n 24), Wirsungo-jejunostomy 26). proportion experiencing similar operated nonoperated patients. both groups correlated disease associated dysfunction. 50% survival (with without surgery) 20-24 (after onset), thus markedly shorter than nonalcoholic pancreatitis. 86 died.
