UK quality statements on end of life care in dementia: a systematic review of research evidence.

摘要: Globally, the number of people who die with dementia is increasing. The importance a palliative approach in care recognised and there are national polices to enhance current care. In UK implementation these promoted by National Institute for Health Care Excellence (NICE) Dementia Quality Standards (QS). Since publication QS new interventions have been developed. To explore critically international research evidence on effect available inform NICE relevant end life (EOL) We used systematic review methods seek three statements within QS. These those that recommend: (1) case management approach, (2) discussing consideration making statement about future (SFC) (3) assessment (PCA). included evaluative studies comparative design, such as trials cohort studies, measured EOL outcomes persons dying moderate severe dementia. Our primary outcome interest was whether intervention led measurable impact wellbeing person their family. assessed quality using scale Higginson colleagues (2002) care, two authors undertook key processes. Data sources Cinahl, Embase, PsychINFO from 2001 August 2014. search strategy free text medical subject headings population recommended found seven evaluating intervention; four SFC, PCA. None management. Studies were weak design; all retrospective data relied others precise record keeping accurate recall events. There limited overlap measurements. Overall reported benefits mixed. useful advance practice however they base. High empirical work needed establish recommendations best practice.
