摘要: AIM: To analyze the factors influencing prognosis of patients with gastric cancer after surgical treatment, in order to optimize procedures. METHODS: A retrospective study 2613 consecutive was performed. Of these patients, 2301 (88.1%) received operations; 196 explorative laparotomy (EL), 130 by-pass procedure (BPP), and 1 975 resection tumors (891 palliative 1084 curative resection). The survival rate calculated by actuarial life table method, prognostic were evaluated using Cox regression proportional hazard model. RESULTS: 2450 (93.8%) followed-up. median period 4.6 mo for without operation, 5.2 EL, 6.4 BPP, 15.2 (P = 0.0001). tumors, overall 1, 3 5-year rates 82.7%, 46.3% 31.1%, respectively, being 51.2% resection, 7.8% those resection. 32.5% total gastrectomy, 28.3% gastrectomy plus adjacent organs. that independently correlated poor included advanced stage, upper third location, differentiation, type IV Borrmann classification, tumor metastasis (N3), invasion into serosa contiguous structure, proximal subtotal carcinoma D1 lymphadenectomy treatment. CONCLUSION: primary lesion should be resected as long local condition permitted stage III prolong patients’ improve their quality operation. Total is indicated carcinomas cardia fundus, involving organs distant requires involved
