Endocardial radiofrequency ablation for hypertrophic obstructive cardiomyopathy: acute results and 6 months' follow-up in 19 patients.

摘要: Objectives The purpose of this study was to examine the efficacy and safety endocardial radiofrequency ablation septal hypertrophy (ERASH) for left ventricular outflow tract (LVOT) gradient reduction in hypertrophic obstructive cardiomyopathy (HOCM). Background Anatomic variability vessels supplying obstructing bulge can limit transcoronary HOCM. Previous studies showed that inducing a local contraction disorder without reducing mass results effective reduction. We examined an alternative approach by using ERASH. Methods Nineteen patients with HOCM were enrolled; 9 patients, septum ablated, 10 right ablated. Follow-up examinations (echocardiography, 6-min walk test, bicycle ergometry) performed 3 days 6 months after Results After 31.2 ± pulses, significant sustained LVOT could be achieved (62% resting gradients 60% provoked gradients, p = 0.0001). walking distance increased significantly from 412.9 129 m 471.2 139 months, 0.019); New York Heart Association functional class improved 3.0 0.0 1.6 0.7 (p Complete atrioventricular block requiring permanent pacemaker implantation occurred 4 (21%); 1 patient had cardiac tamponade. Conclusions ERASH is new therapeutic option treatment HOCM, allowing as well symptomatic improvement acceptable discrete disorder. It may suitable not amenable or myectomy.
